Metamed accepts physician referrals and self-referrals. No formal referral letter is required — but if you're a healthcare provider, sharing context helps us co-ordinate care.
For physicians and allied providers
If you're referring a patient for obesity medicine, metabolic health, cardiovascular risk management, or comprehensive longevity care, please share:
- Most recent bloodwork, including lipids, HbA1c, and TSH if available
- Relevant medication list, particularly diabetes, hypertension, or thyroid medications
- Any active comorbidities or contraindications we should be aware of
A Metamed physician reviews every clinical referral personally and will follow up with the referring provider once the patient is assessed.
For existing patients
If you're a current Metamed patient referring a friend or family member, mention your name in the form below — your coordinator will follow up. We don't run referral incentive schemes, but we deeply appreciate the trust.
What happens after we receive the referral
- Within 1 business day: a plan coordinator reaches out to the prospective patient to schedule the free 20-minute consultation
- Within 2 weeks: if there's a fit, the patient is booked for a physician consultation (typically OHIP-covered)
- Ongoing: the referring provider receives a summary letter after the initial workup, if requested
